Facilities Form
If your group is interested in using FUMC-Hershey
church facilities for meetings or events, please review this Church Use
Application form. Two copies of the application form along with the
Facilities User Agreement must be returned to the church office
preferably two weeks before your event. Rooms fill up fast so please
return the request as soon as possible.
Sensory Sensitivity Inventory
There are individuals in our community that have sensory sensitivities
that may make it challenging for them to participate as fully as they
would wish during worship and other church gatherings. Please fill out
this inventory so we can become more adaptable and accommodating as a
church community.
Alter Flowers Order Form
The alter flowers order form is used to sponsor flower arrangements in
the church sanctuary. Please return to the church office. Flowers cannot
be ordered without payment. Adult Ministry

Student Ministry
Student Medical Release and Permission Form
The Medical Release and Parental Consent Form logs medical history and
insurance information and gives permission for the student to attend all
youth activities sponsored by First United Methodist Church of Hershey.
Music Ministry
Children's Choir Registration Form
The Children's Choir Registration Form is for
registering for Cherub Choir, Chapel Choir, and Children's Handbell Choir.
Children who will be entering K to 5th grade are eligible.
Youth Choir Registration and Covenant Form
The Youth Choir Registration Form is for
registering for Youth Choir or Youth Handbell Choir. Youth entering grades 6-12 are eligible.

Financial Ministry
EFT
Authorization Agreement Form
The EFT Form is used to start electronic giving to FUMC-Hershey. You must complete the form
and provide a voided check. The form and voided check will be retained at the
church office and the information you provide will be uploaded to Vanco, via
their secure website.
Fundraising
and Collection Application Form
The Fundraising and Collection Application form must be completed to facilitate
planning and coordination among all groups. Please review and
complete the form and return it to the church office.
Property Donation Form
The property donation form helps to ensure
that 1) the property donations are added to the property inventory for
insurance purposes, 2) the property can, and will be utilized and, 3)
the proper donor is recognized.
Reimbursement Voucher Form
If you are a church member seeking
reimbursement for ministry expenses, print this Voucher Form and submit
it to John Fisher at the church office.

Form used to make a financial commitment to our Capital Campaign.
Missions
Guidelines for Fundraising and Grants for Short-term Missions and Work Camps
These guidelines help mission participants through the process of financing
mission trips and work camps.
Mission Scholarship Application
This application is to be completed by individuals wishing to receive
scholarship money to go on a mission trip.

Throughout the year, there will be opportunities for active adult
members and their families to raise monies for up-coming mission trips
and opportunities sanctioned or sponsored by the church, mission
committee, or the Susquehanna conference by ushering at local
